The focus of my fourth week was dedicated to knowing the medication used in the ward.<\/p>\n\n\n\n
I had a very fruitful discussion with my mentors discussing the drugs, indications, side effects and the regular lab test used in monitoring.<\/p>\n\n\n\n
-Fluoxetine, Seronil 5mg-20mg<\/p>\n\n\n\n
-Escitalopram, Cipralex 2.5mg-20mg<\/p>\n\n\n\n
-Citalopram, Cipramil, Sepram 10-40mg<\/p>\n\n\n\n
Indications:<\/strong><\/p>\n\n\n\n Indication for the prescription and use of the drugs above includes depression, anxiety, panic disorder, social phobia, obsessive-compulsive disorder, mutism (anxiety).<\/p>\n\n\n\n It is also very important to monitor the following blood tests before and after a patient is prescribed and administered the above drugs. CBC, ALT, EKG (not every time)<\/p>\n\n\n\n -liver metabolism, rarely QT interval, may increase bleeding risk<\/p>\n\n\n\n Common side effects:<\/strong><\/p>\n\n\n\n Headache, restlessness, generally not sedating, exacerbation of suicidal thoughts (warning), (sexual effects)<\/p>\n\n\n\n Interactions:<\/strong><\/p>\n\n\n\n Fluoxetine enhances the effectiveness of risperidone.<\/p>\n\n\n\n 2. Antipsychotics:<\/strong><\/p>\n\n\n\n Risperidone; Risperdal, Risperidone 0.25mg-2mg<\/p>\n\n\n\n Aripiprazole, Abilify 2.5mg-15mg<\/p>\n\n\n\n -olanzapine, Zyprexa 2.5-25mg<\/p>\n\n\n\n Quetiapine, Ketipinor 12.5mg-300mg?<\/p>\n\n\n\n Test to monitor includes the following:<\/strong> CBC, lipids, sugar, EKG<\/p>\n\n\n\n Common side effects:<\/strong><\/p>\n\n\n\n Sedating, especially olanzapine and risperidone<\/p>\n\n\n\n Weight gain with olanzapine, risperidone; less with quetiapine and abilify<\/p>\n\n\n\n fat\/sugar metabolism disorders; least with aripiprazole<\/p>\n\n\n\n Stiffness<\/p>\n\n\n\n Disorganization<\/p>\n\n\n\n Emotional blunting<\/p>\n\n\n\n ADHD (Attention<\/strong>-Deficit\/Hyperactivity Disorder) <\/strong>MEDICATION:<\/strong><\/p>\n\n\n\n ADHD medication is usually considered after a confirmed diagnosis by a qualified mental healthcare professionals. This often involves a thorough evaluation including interviews, questionnaires, and sometimes psychological testing.<\/p>\n\n\n\n Usually, ADHD symptoms include the following;<\/p>\n\n\n\n inattention (6 symptoms)<\/p>\n\n\n\n hyperactivity (3 symptoms), and impulsivity (3 symptoms),<\/p>\n\n\n\n Methylphenidates:<\/strong><\/p>\n\n\n\n Medikinet :<\/strong> 5mg; duration of action 4 hours<\/p>\n\n\n\n Medikinet CR :<\/strong> 10mg-40mg<\/p>\n\n\n\n Equasym retard :<\/strong>10mg-60mg<\/p>\n\n\n\n Concerta :<\/strong>18mg, 27mg, 36mg, 54mg; duration of action 12 hours<\/p>\n\n\n\n Test to monitor when administering Methylphenidates:<\/strong><\/p>\n\n\n\n Blood Pressure and Heart Rate: <\/strong>Regularly check blood pressure and heart rate to monitor for hypertension and tachycardia. These should be measured at baseline (before starting medication), shortly after starting, and periodically during treatment.<\/p>\n\n\n\n Electrocardiogram (ECG):<\/strong> Obtaining an ECG before starting treatment in individuals with a history of heart disease, a family history of sudden death, or symptoms suggestive of cardiovascular disease.<\/p>\n\n\n\n Height and Weight:<\/strong>Regularly track height and weight to monitor for potential growth suppression. This should be done at baseline and at regular intervals<\/p>\n\n\n\n Common Side Effects<\/strong><\/p>\n\n\n\n Appetite Suppression: Leading to weight loss, particularly in children.<\/p>\n\n\n\n Insomnia: Difficulty falling or staying asleep.<\/p>\n\n\n\n Headache: Mild to moderate headaches.<\/p>\n\n\n\n Stomach Pain: Gastrointestinal discomfort, often mild.<\/p>\n\n\n\n Dry Mouth: Feeling of dryness in the mouth.<\/p>\n\n\n\n Nausea: Sometimes accompanied by vomiting.<\/p>\n\n\n\n Increased Heart Rate: Tachycardia.<\/p>\n\n\n\n Increased Blood Pressure: Hypertension.<\/p>\n\n\n\n Other medications for ADHD include the following:<\/p>\n\n\n\n Lisdexamfetamine (Elvanse ):<\/strong> 20mg, 30mg, 50mg, 60mg, 70mg; duration of action 10<\/p>\n\n\n\n hours.<\/p>\n\n\n\n Atomoxetine (Strattera<\/strong> ):10mg, 18mg, 25mg, 40mg, 60mg, 80mg, 100mg<\/p>\n\n\n\n Guanfacine (Intuniv): <\/strong>1-3mg; continuous effect<\/p>\n\n\n\n Side Effects:<\/strong><\/p>\n\n\n\n Insomnia: Difficulty falling asleep or staying asleep.<\/p>\n\n\n\n Decreased appetite: A loss of appetite, which can lead to weight loss.<\/p>\n\n\n\n Dry mouth: Feeling thirsty or having a dry sensation in the mouth.<\/p>\n\n\n\n Headache: Mild to moderate headaches are reported by some users.<\/p>\n\n\n\n Nausea: Feeling sick to your stomach or experiencing stomach discomfort.<\/p>\n\n\n\n Anxiety or nervousness: Increased feelings of anxiety or nervousness.<\/p>\n\n\n\n Irritability: Feeling easily annoyed or agitated.<\/p>\n\n\n\n Increased heart rate or blood pressure: Stimulant medications like lisdexamfetamine can elevate heart rate and blood pressure.<\/p>\n\n\n\n It has being a very good week, all thanks to my mentors for their cooperation. I also hope the last week will present more learning opportunities.
