Paediatric Internship Learning Task
For my Learning task I have chosen a 3-year-old boy, he is a new member in our Keltainenserkut group and he has a speech difficult – stuttering.
From my point of view this case is quite informative, interesting and good-to-know.
Why I made this choice. There are a complex of important paediatric aspects in this case: physical and emotional wellbeing, ethics and legislation. Because, every one person has human rights get equal and good quality education services, i.e. the Finnish Law guarantees the human rights equally for everyone, even the smallest one – Suomen Perustuslaki 731/1999
My general targets:
- help him to get integration into new group comfortable, easy and stressless
- prevent in advance he’s difficulties related to his speech stuttering
- prevent and avoid his difficulties in communicating with the group- mates
- make for him polite, positive and friendly integration by implementing the different age-specific gams and activities
Beforehand
We (my tutor, me and group-colleagues), have arranged a meeting there we had been planing induction and integration plan for him. It was really new and interesting experience for me.
By our discussion I suggested the suitable places, from my point of view, for his eating and sleeping, e.g. I suggested the “Koirat” mini-group for him. Because our Keltainenserkut group is divided onto 3 mini-groups: Koirat, Puput ja Tiikerit. Thus each one group is seating around they own table. Thus, there are sitting one more childcarer (lastenhoitaja) with “Koirat” mini-group.
Additionally, I recommended to use visual aids to communicate with him – Näkyvä Lasten Kortit – Eidetic Memory Cards. My mentor has supported and confirm my idea. My tutor uses a set of Eidetic Memory Cards that adapted especially for 3-year-old children.
This flashcards are one of effective study tool that can be great for preschoolers, child with stuttering, ADHD ect.
This Eidetic Memory Cards develop his speech and communication by the visual signals. Thereby all of this build his confidence and help him in his integration.
After that we had conversation with his mother and provided her information and guidance, answered for her questions.
During
On the first day I have printed plats wit his name and a small picture with dog, because it is easy remember and understandable for him. I shown him our rooms, WC, his slipping and eating place.
During the first day I have been explaining him step-by-step our day routines, helping him to remember the names of group-mates. And in same time I was helping our children to get easy friendship with him.
In other words, on the one hand I was helping him and on the other hand I was helping the rest of the children to reach a well understanding each one.
In order to understand which type of play he like and prefers I have been trying to play with him many different games age-specific and activities.
Thus, it was really good-to-know practice for me. Because there were various situations where I was applying my theory knowledge about age-developing aspects, play-metods for his comfortable adapting and prevent any stress situation.
Interventions:
As a student-nurse I was using a couple nursing interventions that to help him to integrate into our group effectively:
- Speech: work with the child’s parents and the kindergarten staff to explain them the speech difficult – stuttering aspects; took into practice Eidetic Memory Cards to improve his speech and communication.
- Social interactions & Group-mate modelling: with collaborative games, I was motivating him and encouraging our group-mates to engage to positive interaction and communication.
After
I was monitoring his daly activities and his emotional status. I was regularly assessing his progress towards, his social and emotional needs, as well as their developmental level.
With my colleagues we were discussing and analysis how it is going and do we have to do any else.

Self-Evaluation
As a result, he easily and without stress has integreted into new group; he has got excellent communication and friendship with classmates.
And one more thing, that is more important for me, he has got more and more better speaking every day. So his speech skills improved a lot.
Therefor, my nursing care plan is effective and I thing we have achieved a really good result.

ASSIGNMENT FOR INTERNSHIP:
Choose a child patient from your training unit and participate in the child’s care. Find out by different means what kind of play the child prefers. Search evidence-based information on age-specific plays and activities, guidance, preparation of the child for the ward and intervention, as well as significance of hospital experiences for children. Plan and put into practice a situation where you apply your knowledge about play and the use of it in adapting the child for something new, for example preparation of the child for intervention or dealing with the experiences of the child. Write down notes of the situation and self-evaluation of your performance, record the feedback received from the tutor, child/family/colleagues, and references used as well. Enter data on implementation of the assignment in the blog. Include the references used.
