I have already familiarized myself with the residents and the environment, therefore it is easy for me to help and work confidently.<\/p>\n\n\n\n<p>I saw notable improvements in my clinical skill and self-confidence. I improved my capacity to establish deep relationships with the residents by applying empathy to comprehend their requirements and worries. I successfully contributed to the creation and implementation of care plans by putting my theoretical knowledge to use, with an emphasis on fostering patient comfort, mobility, and cognitive engagement. To ensure prompt interventions, I showed diligence in keeping an eye on patients&#8217; health, spotting changes in their conditions, and clearly communicating results to my mentor. I also improved my organizational abilities by effectively setting priorities for activities in a fast-paced care setting.<\/p>\n\n\n\n<p><strong>PROFESSIONAL COMPETENCE AND GOALS<\/strong><\/p>\n\n\n\n<p><strong>LEARNING SKILLS<\/strong><\/p>\n\n\n\n<p>Firstly, I used to put my theoretical knowledge into practice when caring for the patients. Being able to use basic Finnish language and sign languages has assisted my communication with the residents. I managed to build trust with the residents, ensuring a safe environment and active in their care plan. I quickly observe any slight changes I saw in the residents&#8217; habits and health situations. I also put the techniques I had learnt for encouraging movement and mental stimulation into practice by customizing exercises to meet the needs of each person. I was able to modify and enhance my methods by thinking back on each encounter, which promoted residents\u2019 welfare and my professional development.<\/p>\n\n\n\n<p><strong>ETHICAL COMPETENCE AND PROFESSIONALISM<\/strong><\/p>\n\n\n\n<p>Ethical competence and professionalism were maintained by upholding patients&#8217; dignity, keeping information private, and making choices that put their welfare first. I maintained my professionalism by adhering to the unit instructions and guidelines. I made sure patients felt appreciated by actively listening to their needs, standing up for their preferences, and honouring their choices such as refusing to wear certain clothes, thereby fostering the trust and supportive environment necessary for high-quality gerontology care.<\/p>\n\n\n\n<p><strong>CLINICAL NURSING COMPETENCE<\/strong><\/p>\n\n\n\n<p>My clinical nursing competence was applied by conducting a comprehensive assessment of older residents, focusing on their physical, cognitive, and emotional health. I successfully administered medications, monitored vital signs, monitored and gave insulin to diabetes patients, blood samples, and identified potential risks such as falls or pressure ulcers. I coordinated with my mentor to enhance care plans. I showed empathy and patience in addressing patients\u2019 concerns and utilized evidence-based interventions to manage chronic conditions, promote safety and improve the quality of life.<\/p>\n\n\n\n<p><strong>SOCIAL AND HEALTHCARE WORKING ENVIRONMENT AND EVIDENCE-BASED CARE<\/strong><\/p>\n\n\n\n<p>I developed trust and productive relationships with the residents and the nurses by making use of my social and healthcare work environment. I implemented evidence-based care by incorporating best practices like patient-centred care planning, customized mobility support, and fall prevention techniques into routine chores. To improve patient safety and well-being, I made sure that interventions were based on current research after discussing with my instructor. This method reaffirmed how crucial empathy, collaboration, and evidence-based decision-making are to the residents with high-quality care.<\/p>\n\n\n\n<p><strong>EDUCATION AND TEACHING COMPETENCE<\/strong><\/p>\n\n\n\n<p>I used my theoretical knowledge to ensure evidence-based interventions while assessing and meeting the need of geriatric residents. Additionally, I taught families and patients health management techniques with a focus on self-care and prevention. I also contributed to case discussions, fostered a learning environment, and shared thoughts with my mentor.<\/p>\n\n\n\n<p><strong>WORK COMMUNITY COMPETENCE<\/strong><\/p>\n\n\n\n<p>I actively participated in team discussions, exchanged ideas on patient-centred strategies, and gained knowledge from mentors and colleagues. By fostering positive relationships with the residents and staff, I was able to enhance patient care while also fostering an environment that was conducive to learning. I also took part in group activities for the residents, such as taking a stroll and meeting the kindergarteners on Valentine&#8217;s Day to improve their social skills.
