However having received theory classes on basic nursing intervention ,i had to implement it and tried to communicate and take my time with the client.<\/p>\n\n\n\n<p>Evaluation:<\/p>\n\n\n\n<p>On evaluation , my tutor Nurse was with me and said it was a good one.<\/p>\n\n\n\n<p>Analysis:<\/p>\n\n\n\n<p>on my reflection while we were with the patient i was able to interact with the patient also being able to carry him along ,informing him on what i was doing&nbsp; made me to gain confidence in myself and reflect as a nurse that while practicing i need to always go back to my book to keep me informed and updated as well as doing the right thing.<\/p>\n\n\n\n<p>Conclusion:<\/p>\n\n\n\n<p>my conclusion is that if I find myself in that situation again i will handle it much better than i did, taking note of what my tutor nurse said and also remembering the positive comment will help build my confidence in caring out the plan .<\/p>\n\n\n\n<p>Action plan:<\/p>\n\n\n\n<p>my action plan in the future is that I be more relaxed and try to plan better on how to carry out the plan . also will like to prepare the things that will be needed to execute the my plan.<\/p>\n\n\n\n<p>Week 2 :Things that i did in this week.<\/p>\n\n\n\n<p>Bed cleaning<\/p>\n\n\n\n<p>Bathing and bed sheet changing<\/p>\n\n\n\n<p>Feeding.<\/p>\n\n\n\n<p>giving patients medicine<\/p>\n\n\n\n<p>having carried out all this duties in week 2 , i would like to base my reflection of learning on the general side of my learning<\/p>\n\n\n\n<p>Description :<\/p>\n\n\n\n<p>getting acquainted with the clients is a vital part of nursing and also with the people working there as it makes the job more harmonious.<\/p>\n\n\n\n<p>Feeling :<\/p>\n\n\n\n<p>In this 2 weeks, i have had different feelings ,since we all have different methods of performing our duties but one key fact that i have noticed is that the general principle of nursing is always followed.<\/p>\n\n\n\n<p>Evaluation :<\/p>\n\n\n\n<p>while administering the medication to a client my mentor nurse made me calm and asked me to relax and was with me all through the way, this made me really calm and wanting to do more.<\/p>\n\n\n\n<p>Analysis :<\/p>\n\n\n\n<p>before i carried the plan out i had to think of how to carry my plan and successful so i tried to understand the process of what i was doing and make the patient to know my plan so that&nbsp; the patient can be calm.<\/p>\n\n\n\n<p>Conclusion :<\/p>\n\n\n\n<p>My conclusion is that i should always know what am about to do and always ask for advice and support on what to do by my tutor nurse.<\/p>\n\n\n\n<p>Action Plan :<\/p>\n\n\n\n<p>to try to be calm at always<\/p>\n\n\n\n<p>Week 3.<\/p>\n\n\n\n<p>Everyday morning duties such as<\/p>\n\n\n\n<p>Bed cleaning of patients , morning shower, feeding patients.<\/p>\n\n\n\n<p>Giving patients medication<\/p>\n\n\n\n<p>doctor visit<\/p>\n\n\n\n<p>Putting patients medicine into dosetti<\/p>\n\n\n\n<p>Documentation<\/p>\n\n\n\n<p>Changing bed patients position<\/p>\n\n\n\n<p>Description:<\/p>\n\n\n\n<p>carrying out morning routines with the patients like bed cleaning checking of bed wound and redness on the skin .also feeding the patient and assisting patient to take their bath either on the bathing wheelchair or&nbsp; bathing bed ,giving medications to the patient making sure that they take in their medication.<\/p>\n\n\n\n<p>learning how to use Hilkka, understood the method how medications are been arranged for each client in the home, had a meeting with the visiting doctor with a client .<\/p>\n\n\n\n<p>Feeling:<\/p>\n\n\n\n<p>while carrying out this plan i appreciated the human body more , knowing that daily cleaning of the skin keeps the skin healthy and free from skin diseases&nbsp; and allows me as a nurse to monitor the patient skin if or when there is any skin ulcer, seeing how the skin changes as we go in life .this has made me understand the precision taking when it comes to patients medication. been with the doctor, also made me understand how the opinion of a mentally stable client is respected, no matter where they are or the condition they find themselves in and this conversation was carried on with a close family member.<\/p>\n\n\n\n<p>Evaluation:<\/p>\n\n\n\n<p>during my evaluation ,my tutor nurse encouraged me to take time to when handling patients medication (by always double checking), include patients in the there daily routine .<\/p>\n\n\n\n<p>ANALYSIS :<\/p>\n\n\n\n<p>Before i carry out my work description i always try to plan on how to make it a success ,this allows the patients to be clam .<\/p>\n\n\n\n<p>Conclusion:<\/p>\n\n\n\n<p>my conclusion is to concentrate&nbsp; on my patients skin, feelings when attending to them and also be very careful and be patient.<\/p>\n\n\n\n<p>Action:<\/p>\n\n\n\n<p>my action plan is that concerning&nbsp; bed cleaning and body wash is that i have to concentrate on the body especially drying up the body after bathing to avoid fungi growth around the hidden areas of the body.<\/p>\n\n\n\n<p>Week 4<\/p>\n\n\n\n<p>week 4 was about working independently on my own and nurses own time with patients.<\/p>\n\n\n\n<p>Medication giving<\/p>\n\n\n\n<p>nightly routine check up<\/p>\n\n\n\n<p>Description<\/p>\n\n\n\n<p>working&nbsp; independently with the patients on my own ,doing morning routines such as morning wash, evening wash, feeding and medication handout. i was instructed to carry out this plan by my tutor nurse .<\/p>\n\n\n\n<p>Feelings :<\/p>\n\n\n\n<p>Carry out my morning routine on my own wasn\u2019t difficult but support the patients out of bed was beginning to feel more like an understanding between myself and the clients<\/p>\n\n\n\n<p>Evaluation :<\/p>\n\n\n\n<p>my tutor nurse came in to check if everything was going on well with me and also asking patients if they where also OK. and after wards i have to evaluate their situation and give my tutor nurse&nbsp; feedback of situation such as what was&nbsp; wrong the patient and as a student nurse what do i think is the cause and how do we help the patient and my tutor was really pleased with me .<\/p>\n\n\n\n<p>Analysis:<\/p>\n\n\n\n<p>I feel and think that the patients allowing me to carry out this plan means that they trust me to it successfully therefore allowing me to gain more knowledge and confidence as a student nurse.<\/p>\n\n\n\n<p>Conclusion:<\/p>\n\n\n\n<p>my conclusion to my week 4 activities would be that practicing more will allow me to be perfect and allowing me to think of other means or ways to carry out the plan quick and in a safe manner.<\/p>\n\n\n\n<p>Action:<\/p>\n\n\n\n<p>My action plan will be to get more knowledge and to keep myself up to date to enable me carry out the plan successfully and for me to be able to inform the patients about the procedure and it benefits and side effects .<\/p>\n\n\n\n<p>Week 5<\/p>\n\n\n\n<p>worked independently and also assisting other health care workers.<\/p>\n\n\n\n<p>Description:<\/p>\n\n\n\n<p>worked independently on my own doing the daily routines morning and afternoon and practicing the nursing basic care clinical skills and also getting to know the patients at this point makes it easy for me to carry out the nursing care effectively.<\/p>\n\n\n\n<p>Feeling :<\/p>\n\n\n\n<p>carrying out my nursing skills independently made me happy with myself and also boosting my confidence and made me to realize that my tutor nurse me.<\/p>\n\n\n\n<p>Evaluation :<\/p>\n\n\n\n<p>carry out this plan and working independently with my tutor nurse supervision and support allows me to practice the clinical skills with confidence and able to carry it out successfully.<\/p>\n\n\n\n<p>Action:<\/p>\n\n\n\n<p>TO CONTINUE TO PRACTICE MORE IN MY NEXT INTERNSHIP.<\/p>\n\n\n\n<p>Conclusion:<\/p>\n\n\n\n<p>To update myself by reading more and keep practicing the clinical skills in future.<\/p>\n\n\n\n<p>GOAL<\/p>\n\n\n\n<p>Summary of learning in my basic nursing care and my Goals.<\/p>\n\n\n\n<p><strong><em>Learning skills<\/em><\/strong>.<\/p>\n\n\n\n<p>I having&nbsp; put some of&nbsp; my theoretical knowledge into&nbsp; practice such as the responsibility of a nurse in care giving, client privacy right, respect to co-workers, punctuality and time consciousness, obeying organizational rules.<\/p>\n\n\n\n<p><strong>Goal to be achieved in my next internship<\/strong>.<\/p>\n\n\n\n<p>Is for me to practicalize&nbsp;my documentation skills in the workplace.<\/p>\n\n\n\n<p><strong><em>Clinical skills:<\/em><\/strong><\/p>\n\n\n\n<p>Having achieved some basic nursing care clinical skills allows me to carry out the basic nursing care as a nursing student , having learned the clinical skills in theory and practicing on manikins in school and during the internship am faced with the real life situations helps me to understand better what i have being taught. hence allowing me to achieve some of my goals ,and those which i was not able to practice i hope to do them in my next internship.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Personal hygiene, clothing\/dressing<\/li>\n\n\n\n<li>Urinary and Bowel elimination, urinary output, catheterization<\/li>\n\n\n\n<li>Skin integrity<\/li>\n\n\n\n<li>Positioning care<\/li>\n\n\n\n<li>Cardinal vital signs (temperature pulse blood pressure, respiration, oxygen saturation, pain, blood sugar)<\/li>\n\n\n\n<li>Bed washing and bed dressing<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><em>Goals to be achieved in my next internship<\/em><\/strong><\/li>\n\n\n\n<li>pharyngeal and nasopharyngeal suctioning<\/li>\n\n\n\n<li>The use of ABCDE-method to examine patient\u2019s vital sings and ISBAR-method in reporting<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Breathing, oxygen therapy (respiratory frequency, SPo2)<\/li>\n\n\n\n<li>Oral care<\/li>\n\n\n\n<li>Fluid balance (fluid chart\/urine output)<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Teaching and Guidance<\/h2>\n\n\n\n<p>Having learned to guide&nbsp; patient with memory loss and also guiding them and other patients through their daily actives and talking them through their medication .as well as rehabilitative nursing care and evaluating my patient\u2019s guidance<\/p>\n\n\n\n<p><strong>Goal to be achieved in my next internship.<\/strong><\/p>\n\n\n\n<p>To guide patient relative<\/p>\n\n\n\n<p>To also guide my patients with nutritional care.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Patient safety and quality of social and health care services<\/strong><\/h2>\n\n\n\n<p>I have&nbsp; learned to anticipate incidents in different work environments and pay attention to patient safety which includes physical, mental and social security of patients (fall risk and prevention) ,i also have knowledge on how to give report to my supervisor concerning patient well being and independently seek more information regarding patient safety in nursing care. therefore i would still like to achieve some of my goals in my next internship.<\/p>\n\n\n\n<p><strong>Goal to be achieved :<\/strong><\/p>\n\n\n\n<p>Factors affecting medication safety (Haipro-system).<\/p>\n\n\n\n<p>ABCDE-method in patient\u2019s examination<\/p>\n\n\n\n<p>ISBAR method of reporting<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Working community Competence<\/strong><\/h2>\n\n\n\n<p>I was able to work as part of the nursing team this allowed me to have the Finnish&nbsp; My mentor nurse, has tutored me on several techniques on the various ways to remember the tools used. Also, hand disinfection and sterile equipment been kept sterile has been also the focus of our practice in the first week.<\/p>\n\n\n\n<p>Feeling :<\/p>\n\n\n\n<p>It was quite astonishing at first, to see everything in plane sight and close proximity, the time it takes and the effort everyone puts into in every surgery in my first week. it made me understand he proper essence of team work and team understanding, as during the process you get to understand that even without spoken words, communication is passed on. done and the communication route between an anesthesia nurse, anesthesia doctor and other surgical team that makes the procedure move smoothly.<\/p>\n\n\n\n<p>understanding of the vital signs and different parameters for different patients has been explained adequately to me, thereby making me able to follow on every patient that undergoes a certain procedural process.<\/p>\n\n\n\n<p>I was able to follow up on the medication given to each patient and also monitoring the medications, thereby giving the opportunity to act as soon as possible.<\/p>\n\n\n\n<p>i have understood virtually, the patient i felt empathy for the client ,I was worried about the fragility of the babies and was concerned about not hurting or braking them but my mentor nurse gave me the confidence to be able to level in premature and my mentor nurse introduced me to this and also how to utilize them and how to also read the readings.<\/p>\n\n\n\n<p>i have learnt different techniques on how to carry and position the baby in other to make them feel comfortable and reduce their pain to barest minimum without needing to interrupt sleep or rest.<\/p>\n\n\n\n<p>Feeling :<\/p>\n\n\n\n<p>i feel much i have understood the different timing of when and why we have to give the medications are does specific timing as interaction of drugs are very important in the human body.<\/p>\n\n\n\n<p>bathinfg the baby under the sink has given me a broadeer view of difference between lappupesu and sink washing, as i have seen and understood that most babies tend to be calm and enjoy the feeling of the flow of warm water on their body.<\/p>\n\n\n\n<p>feeding is a very important role with NICU babies and also for babies in general but what is more important is feeding properly and with the right direction. with the help of my mentor nurse she has directed me and guided me in how to feed, timing of when to let the baby breath using diferent technique and i know this is helpful for my future career.<\/p>\n\n\n\n<p>my mentor nurse has been able to make me understand how we treat parents in relation to how we treat our patients (which are NICU babies), i have understood in as much as we ward responsibilities, as i had time to try various things and also see something totally different from the ward. i was given the opportunity to go with one of our babies to the surgical room, to observe how a new tracheostomy is been replaced in such type of environment and as such giving me a proper understanding of precision and timing when it comes to dealing with prematures.<\/p>\n\n\n\n<p>I assisted in the preparation of a baby for surgery and also taking the baby to the surgical room with other health care professionals and assisted in returning the baby back to ICU ward of NICU and preparation of post operative surgery for the baby in our ICU room.<\/p>\n\n\n\n<p>i having been able to give medication through PICC and CVK under supervision of nurses and this has gone properly well, especially because i have learnt from their method s of how it is done and also applications from my previous internship.<\/p>\n\n\n\n<p>in this week one of the most intense thing was the retinopathy, as i also witnessed how that was done and i never imagined that such type of procedures are been carried out on babies despite knowing that this are one of the abnormalities associated in the retinal vascular development that occurs in premature infants.<\/p>\n\n\n\n<p>I have been in the medication room and assisted in medication calculation and i have seen how precise and accurate things are done, also the sensitivity of how a little mistake could be hazardous and disaster to baby.<\/p>\n\n\n\n<p>Feelings :<\/p>\n\n\n\n<p>the whole situation has been very new, interesting little like in the movies especially in the surgery room but as well, i love the experience.<\/p>\n\n\n\n<p>I understand on a daily bases , more and more the very importance of Olen saanut paremman ymm\u00e4rryksen lasten onkologian osaston ja p\u00e4iv\u00e4osaston p\u00e4ivitt\u00e4isest\u00e4 rutiinista ja alkanut sopeutua siihen. Valmistelin potilaiden saapumista varmistaen, ett\u00e4 jokaisessa huoneessa oli kaikki tarvittavat v\u00e4lineet, ja tein potilaista olonsa mukavan.
