<\/figure>\n\n\n\nNURSING ASSESSMENT AND INTERVENTION<\/strong><\/p>\n\n\n\n week 1<\/p>\n\n\n\n The things i did during my first week of internship are :<\/p>\n\n\n\n checking of ulcer or bed wound on patients skin .<\/p>\n\n\n\n changing of patient`s cloth and bedding .<\/p>\n\n\n\n Body wash and feeding<\/p>\n\n\n\n Description :<\/p>\n\n\n\n My tutor nurse and I was with different clients to assess them and take care of them and I was also asked to do the same as was done by the tutor nurse and I was allowed to start feeding patient.<\/p>\n\n\n\n Feeling :<\/p>\n\n\n\n While i was with the patient i felt confident and felt empathy for the client ,I was worried about the language barrier and how to communicate with the older patients. However having received theory classes on basic nursing intervention ,i had to implement it and tried to communicate and take my time with the client.<\/p>\n\n\n\n Evaluation:<\/p>\n\n\n\n On evaluation , my tutor Nurse was with me and said it was a good one.<\/p>\n\n\n\n Analysis:<\/p>\n\n\n\n on my reflection while we were with the patient i was able to interact with the patient also being able to carry him along ,informing him on what i was doing made me to gain confidence in myself and reflect as a nurse that while practicing i need to always go back to my book to keep me informed and updated as well as doing the right thing.<\/p>\n\n\n\n Conclusion:<\/p>\n\n\n\n my conclusion is that if I find myself in that situation again i will handle it much better than i did, taking note of what my tutor nurse said and also remembering the positive comment will help build my confidence in caring out the plan .<\/p>\n\n\n\n Action plan:<\/p>\n\n\n\n my action plan in the future is that I be more relaxed and try to plan better on how to carry out the plan . also will like to prepare the things that will be needed to execute the my plan.<\/p>\n\n\n\n Week 2 :Things that week 2 , i would like to base my reflection of learning on the general side of my learning<\/p>\n\n\n\n Description :<\/p>\n\n\n\n getting acquainted with the clients is a vital part of nursing and also with the people working there as it makes the job more harmonious.<\/p>\n\n\n\n Feeling :<\/p>\n\n\n\n In this 2 weeks, i have had different feelings ,since we all have different methods of performing our duties but one key fact that i have noticed is that the general principle of nursing is always followed.<\/p>\n\n\n\n Evaluation :<\/p>\n\n\n\n while administering the medication to a client my mentor nurse made me calm and asked me to relax and was with me all through the way, this made me really calm and wanting to do more.<\/p>\n\n\n\n Analysis :<\/p>\n\n\n\n before i carried the plan out i had to think of how to carry my plan and successful so i tried to understand the process of what i was doing and make the patient to know my plan so that the patient can be calm.<\/p>\n\n\n\n Conclusion :<\/p>\n\n\n\n My conclusion is that i should always know what am about to do and always ask for advice and support on what to do by my tutor nurse.<\/p>\n\n\n\n Action Description:<\/p>\n\n\n\n carrying out morning routines with the patients like bed cleaning checking of bed wound and redness on the skin .also feeding the patient and assisting patient to take their bath either on the bathing wheelchair or bathing bed ,giving medications to the patient making sure that they take in their medication.<\/p>\n\n\n\n learning how to use Hilkka, understood the method how medications are been arranged for each client in the home, had a meeting with the visiting doctor with a client .<\/p>\n\n\n\n Feeling:<\/p>\n\n\n\n while carrying out this plan i appreciated the human body more , knowing that daily cleaning of the skin keeps the skin healthy and free from skin diseases and allows me as a nurse to monitor the patient skin if or when there is any skin ulcer, seeing how the skin changes as we go in life .this has made me understand the precision taking when it comes to patients medication. been with the doctor, also made me understand how the opinion of a mentally stable client is respected, no matter where they are or the condition they find themselves in and this conversation was carried on with a close family member.<\/p>\n\n\n\n Evaluation:<\/p>\n\n\n\n during my evaluation ,my tutor nurse encouraged me to take time to when handling patients medication (by always double checking), include patients in the there daily routine .<\/p>\n\n\n\n ANALYSIS :<\/p>\n\n\n\n Before i carry out my work description i always try to plan on how to make it a success ,this allows the patients to be clam .<\/p>\n\n\n\n Conclusion:<\/p>\n\n\n\n my conclusion is to concentrate on my patients skin, feelings when attending to them and also be very careful and be patient.<\/p>\n\n\n\n Action:<\/p>\n\n\n\n my action plan up<\/p>\n\n\n\n Description<\/p>\n\n\n\n working independently with the patients on my own ,doing morning routines such as morning wash, evening wash, feeding and medication handout. i was instructed to carry out this plan by my tutor nurse .<\/p>\n\n\n\n Feelings :<\/p>\n\n\n\n Carry out my morning routine on my own wasn\u2019t difficult but support the patients out of bed was beginning to feel more like an understanding between myself and the clients<\/p>\n\n\n\n Evaluation :<\/p>\n\n\n\n my tutor nurse came in to check if everything was going on well with me and also asking patients if they where also OK. and after wards i have to evaluate their situation and give my tutor nurse feedback of situation such as what was wrong the patient and as a student nurse what do i think is the cause and how do we help the patient and my tutor was really pleased with me .<\/p>\n\n\n\n Analysis:<\/p>\n\n\n\n I feel and think that the patients allowing me to carry out this plan means that they trust me to it successfully therefore allowing me to gain more knowledge and confidence as a student nurse.<\/p>\n\n\n\n Conclusion:<\/p>\n\n\n\n my conclusion to my week 4 activities would be that morning and afternoon and practicing the nursing basic care clinical skills and also getting to know the patients at this point makes it easy for me to carry out the nursing care effectively.<\/p>\n\n\n\n Feeling :<\/p>\n\n\n\n carrying out my nursing skills independently made me happy with myself and also boosting my confidence and made me to realize that my tutor nurse of biopsy from different patients. distribution and cross-checking of medication for several patients, to ensure that each of them has the right medication that has been prescribed by the doctor. i have fully began to document the activities of different patient in the ward and also documentation of their physically wellbeing<\/p>\n\n\n\n Feeling:<\/p>\n\n\n\n This has broaden my horoscope in the cannulation and also understood the concept of biopsy taking. also i have felt the essence of been careful and double checking in medication.<\/p>\n\n\n\n Evaluation:<\/p>\n\n\n\n during my My mentor nurse, has tutored me on several techniques on the various ways to remember the tools used. Also, hand disinfection and sterile equipment been kept sterile has been also the focus of our practice in the first week.<\/p>\n\n\n\n Feeling :<\/p>\n\n\n\n It was quite astonishing at first, to see everything in plane sight and close proximity, the time it takes and the effort everyone puts into in every surgery in my first week. it made me understand he proper essence of team work and team understanding, as during the process you get to understand that even without spoken words, communication is passed on. My mentor nurse has been able to guide me this first week and also mentor me with patience and understanding.<\/p>\n\n\n\n Evaluation:<\/p>\n\n\n\n On evaluation , my mentor Nurse was with me and has given me good feedback and i intend to keep working hard on myself during this internship period..<\/p>\n\n\n\n Analysis:<\/p>\n\n\n\n on my reflection while we have been working on identify each tools in that are been used for each operation i have understood that sometimes the pattern are similar and with time it will get to you.<\/p>\n\n\n\n Conclusion:<\/p>\n\n\n\n my conclusion, i appreciate the immediate feedback and support from my mentor nurse and how the different staff members are willing to help me grow professionally, also, i think that as time goes on i will gain more confidence and more know how on how to handle things.<\/p>\n\n\n\n Action plan:<\/p>\n\n\n\n My action plan in the future is that I will be away to take on more responsibilities with the monitoring of my mentor nurse and also identification of tools properly.<\/p>\n\n\n\n Week 2<\/strong><\/p>\n\n\n\n Description :<\/p>\n\n\n\n I have been able to scrub in and make couple of mistakes still while wearing the double gloves the easiest way but my mentor nurse has been very helpful in this. i have been able to work aseptically and minimize the feeling of trying to put my hands down wile putting on the sterile gown.<\/p>\n\n\n\n I have been able to assist other scrub nurses in giving them different equipment and tools that could and will be used during the procedures and doing this has also made me feel that i can be trusted by the staff of the ward.<\/p>\n\n\n\n Been a scrub nurse or an operative nurse requires that you gather knowledge of the procedure and what might happen during the procedure and understand the function of everyone who is there as well. My mentor nurse has been rally informative and educating me on every procedure and this has made every procedure feel like i have been in the planning process, as i understand what is expected of each person before, during and after the procedure.<\/p>\n\n\n\n Feeling :<\/p>\n\n\n\n i feel much comfortable at this moment and glad that both my mentor nurse are giving me the guidance and opportunity anesthesia nurse, anesthesia doctor and other surgical team that makes the procedure move smoothly.<\/p>\n\n\n\n understanding of the vital signs and different parameters for different patients has been explained adequately to me, thereby making me able to follow on every patient that undergoes a certain procedural process.<\/p>\n\n\n\n I was able to follow up on the medication given to each patient and also monitoring the medications, thereby giving the opportunity to act as soon as possible.<\/p>\n\n\n\n i have understood virtually, the most important medications needed for a surgical different clients to assess them and take care of them and I was also asked to do the same and take some responsibilities in understanding measurement of different patients, understanding what it means to be breastfeed and to be feed by NG tube .<\/p>\n\n\n\n Feeling :<\/p>\n\n\n\n While i was with the patient i felt empathy for the client ,I was worried about the fragility of the babies and was concerned about not hurting or braking them but my mentor nurse gave me the confidence to be able to hold and access them.<\/p>\n\n\n\n Evaluation:<\/p>\n\n\n\n On evaluation , my mentor Nurse was lappupesu and sink washing, as i have seen and understood that most babies tend to be calm and enjoy the feeling of the flow of warm water on their body.<\/p>\n\n\n\n feeding is a very important role with NICU babies and also for babies in general but what is more important is feeding properly and with the right direction. with the help of my mentor nurse she has directed me and guided me in how to feed, timing of when to let the baby breath using diferent technique and i know this is helpful for my future career.<\/p>\n\n\n\n my mentor nurse has been able to make me understand how we treat parents in relation to how we treat our patients (which are NICU babies), i have understood in as much as we treat the family as a whole entity, our major concern and priority is our patientthe NICU Description<\/p>\n\n\n\n The fourth week was a change in normal NICU ward responsibilities, as i had time to try various things and also see something totally different from the ward. i was given the opportunity to go with one of our babies to the surgical room, to observe how a new tracheostomy is been replaced in such type of environment and as such giving me a proper understanding of precision and timing when it comes to dealing with prematures.<\/p>\n\n\n\n I assisted in the preparation of a baby for surgery and also taking the baby to the surgical room with other health care professionals and assisted in returning the baby back to ICU ward of NICU and preparation of post operative surgery for the baby in our ICU room.<\/p>\n\n\n\n i having been able to give medication through PICC and CVK under supervision of nurses and this has gone properly well, especially because i have learnt from their method s of how it is done and also applications from my previous internship.<\/p>\n\n\n\n in this week one of the most intense thing was the retinopathy, as i also witnessed how that was done and i never imagined that such type of procedures are been carried out on babies despite knowing that this are one of the abnormalities associated in the retinal vascular development that occurs in premature infants.<\/p>\n\n\n\n I have been in the medication room and assisted in medication calculation and i have seen how precise and accurate things are done, also the sensitivity of how a little mistake could be hazardous and disaster to baby.<\/p>\n\n\n\n Feelings :<\/p>\n\n\n\n the whole situation has been very new, interesting little like in the movies especially in the surgery room but as well, i love the experience.<\/p>\n\n\n\n I understand on a daily bases , more and more In the beginning of the internship i came with an open mind to improve on what i have learnt from school and to embrace the learning skills of professionals in the working field.<\/p>\n\n\n\n
I understand that even when a patient is on the surgical table either on general anesthesia or local, the right and privacy of that patient must be respected at all times and also the dignity of the patient.<\/p>\n\n\n\n
I have worked and enjoyed working in this environment and the staffs has been cordial and helpful, for this i can say i have grown very well in this field of studies.<\/p>\n\n\n\n
The patients has and its always my first concern and i have ensured to put their concern first before any other, also considering that every patient is different and has an individual need and as such must be treated as an individual.<\/p>\n\n\n\n
I have been able to make research on my own and also read couple of booklets here in the ward given to me by my mentor, also my mentor has explained holistically through the process of been in the anesthesia side or as an instrument nurse and this has helped me grow.<\/p>\n\n\n\n
I can gladly say that my clinical skills has improved radically and this is a huge thanks to the staffs of the department and especially my mentors for their flexibility and willingness to let me try new things.<\/p>\n\n\n\nLearning Reflection<\/h1>\n\n\n\n

Valmistelin potilaiden saapumista varmistaen, ett\u00e4 kaikki tarvittavat v\u00e4lineet olivat saatavilla jokaisessa huoneessa ja potilaat tunsivat olonsa mukavaksi. Tein yhteisty\u00f6t\u00e4 eri terveydenhuollon ammattilaisten kanssa varmistaakseni, ett\u00e4 lasten p\u00e4ivitt\u00e4inen el\u00e4m\u00e4 ei h\u00e4iriintyisi.

Olen saanut paremman ymm\u00e4rryksen lasten onkologian osaston ja p\u00e4iv\u00e4osaston p\u00e4ivitt\u00e4isest\u00e4 rutiinista ja alkanut sopeutua siihen. Valmistelin potilaiden saapumista varmistaen, ett\u00e4 jokaisessa huoneessa oli kaikki tarvittavat v\u00e4lineet, ja tein potilaista olonsa mukavan. Tein yhteisty\u00f6t\u00e4 eri terveydenhuollon ammattilaisten kanssa varmistaakseni, ettei lasten p\u00e4ivitt\u00e4inen el\u00e4m\u00e4 h\u00e4iriintynyt.
