I already this task in my previous internships  and  I practiced again at my internship unit. Preparing  a dose tray for patients involves organizing and arranging medications to ensure safe and accurate administration. Here&#8217;s a step-by-step guide to preparing a dose tray:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Gather Information:<\/strong><\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Review each patient&#8217;s medication orders, including dosage, timing, and special instructions.<\/li>\n<\/ul>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Organize Materials:<\/strong><\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Collect all necessary supplies, such as medication cups, syringes, pill crushers, gloves, and a pen.<\/li>\n\n\n\n<li>Ensure you have a clean, well-lit workspace to prepare the medications.<\/li>\n<\/ul>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Verify Medications:<\/strong><\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Double-check the medication orders against the patient&#8217;s chart for accuracy.<\/li>\n\n\n\n<li>Look for any recent changes in medication orders.<\/li>\n<\/ul>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Check Expiry Dates:<\/strong><\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Verify the expiration date on each medication to ensure it is safe to use.<\/li>\n<\/ul>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Prepare Medications:<\/strong><\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Measure liquid medications using appropriate syringes or cups.<\/li>\n\n\n\n<li>Count and place pills in medication cups or a pill organizer.<\/li>\n\n\n\n<li>Crush medications if required, using a pill crusher, and mix with the prescribed medium.<\/li>\n<\/ul>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Label Medications:<\/strong><\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Label each medication cup or container with the patient&#8217;s name, medication name, dose, and time of administration.<\/li>\n<\/ul>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Arrange on Tray:<\/strong><\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Arrange the medications on the dose tray in the order they will be administered.<\/li>\n\n\n\n<li>Separate medications for different patients to avoid confusion.<\/li>\n<\/ul>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Double-Check:<\/strong><\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Recheck each medication against the patient\u2019s orders to ensure accuracy.<\/li>\n\n\n\n<li>Confirm patient-specific details, such as allergies or special instructions.<\/li>\n<\/ul>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Prepare Documentation:<\/strong><\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Have a copy of the medication administration record (MAR) ready for documentation.<\/li>\n<\/ul>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Hygiene and Safety:<\/strong>\n<ul class=\"wp-block-list\">\n<li>Wear gloves and follow hygiene protocols to prevent contamination.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Transport Safely:<\/strong>\n<ul class=\"wp-block-list\">\n<li>Ensure the dose tray is stable and medications are secure for transport to the patient area.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n<p>Following these steps helps ensure that medications are administered accurately and safely, minimizing the risk of errors.<\/p>\n\n\n\n<p>My mentor checked it and it was ok.<\/p>\n\n\n\n<p><\/p>\n","protected":false},"excerpt":{"rendered":"<p>In this week I had oppurtunity to prepare weekly medicine tray.
