Final Evaluation of Nursing Assessment and Intervention practice

Learning skills:

I have written the blog about the weekly reflection (from weeks 1 – 4), it is about what things I have learned during my internship and what I can do in developing my competence. In doing work, I try to ask feedback and ask work-related questions, especially the reason behind them, for example, how to give medication properly, the position for feeding patient to prevent reflux, what nutrition is suitable for the elderly ( what is the indication of additional protein drink). Also, I try to combine the theoretical knowledge that I have learned in school and practice. Sometimes, we can not fully apply the theory in a real situation because every patient is unique, so we have to do different approaches.

Ethical knowledge and professionality

During my internship, I always try to act professionally toward workers, patients, and relatives. And I recognize and apply the ethical aspects such as autonomy, beneficence, justice, and non-maleficence. for example, If we want to change clothes, we have to keep privacy (close the door, close the window, close the curtains) and always introduce ourselves to patients, explain what we are going to do and ask permission. we can not force the patient to do something, we have to respect their decision. I also assure the safety of patients especially the elderly who got dementia, they tend to be careless with themselves and high risk to fall down. 

Clinical skills

I measure the vital sign of the patients (blood pressure, heart rate, temperature, oxygen saturation, pain, breath), and I also use a stethoscope to hear their breathing sounds. I tried to do auscultation on an asthma patient. I practiced ABCDE airway, breathing, circulation, disability, and exposure). For hygiene matters, I demonstrate the aseptic technique, it is very important because we don’t know what kind of microorganisms that is surrounding us and will harm the patients if we don’t pay attention to the aseptic technique. Every time I do the procedure,  I work ergonomically because it can reduce the risk factor, increase productivity and prevent discomfort. I take and distribute medication to patients under supervision (asthma drugs, pain killers, laxative drugs, dyspepsia drugs) and pay attention to the “7 rights” of medication administration: right patient, right drug, right dose, right time, right route, right reason, and right documentation. I have presented the E-medication passport to my supervisors and got feedback from them.

Teaching and guidance

During my internship, I am active in helping to guide patients and act professionally. The main barrier is language, but I try to speak more words to them and ask for the correct Finnish words from my supervisor. I realize that I am dealing with the elderly, so i need to take the rehabilitative approach, I assure them to make effort to do the basic needs independently because it is good for improving their outcome.

Quality and safety of social and health care services

Safety is crucial for the elderly. They tend to fall down and imbalanced, especially for the elderly who got dementia. So in everything that I do, I will make sure safety first ( set the bed rails, lap belts in a wheelchair, wearing shoes while walking, lower bed position before leaving, etc). and for some patients who use the patient lifting machine, we need to make sure the position, the belts, and how to operate it, it is important for the patients and the comfort. and when we transfer the bedridden patient to take shower, we need to pay attention to position, adjusting the height and the bedrails. 

Working community skills

I feel very welcomed in this workplace. And I try to help other workers work so I can learn from lots of workers and I can apply the best way for patient outcomes in the future. We communicate with each other and I feel that I become their member. Teamwork is very important and during my shift, I always try to communicate what I am going to do. For example, if we wanna change diapers, I say that I will take the part in wiping and changing clothes and my workmate can do other things. 

Conclusion

I think I have achieved my goals. There are still some areas that I need to practice for example drug calculation, documentation, and reporting. But, I always ask for feedback from my supervisor and they give some tips and I learn a lot from the mistakes that I have made. And remembering the medication is quite difficult because most of them are in Finnish, but I think it takes time. the more I practice, the more I get used to it and be confident when I do it.

    Going through your entire nursing intervention internship, it is evident that you have developed week by week, gaining new skills and competencies and working through the challenges, and gaining valuable experience that key in your career development.

    Taking into consideration that this was your first internship in the Finnish healthcare system, it is encouraging that you have been able to actively put the knowledge into practice effectively, faced by challenges language and culture, this is outstanding.

    You have taken a keen and active role in your learning process, actively seeking information and putting them into practice, receiving feedback and applying them to improve your learning and competencies, this is quite remarkable, keep this up into your next internship and into the working life.

    During this internship, you have also been able to hone your skills in assisting the elderly activities of daily living , carried out monitoring of vital signs, provided medication safely, and participated in different interventions carried out in your unit. It is also clear that you have good hand skills, which is a crucial in carrying out care interventions. I am also glad that you understand your strengths and areas to develop in the next internship.

    Taking a critical look through the weeks that you have been able to carry out your internship in at the nursing home, it is evident that you achieved the goals set out for this internship and the goals that you had set out for yourself.

    Keep up the exceptional work, and the good qualities that you have gained in this internship, and continue to practice interventions that you were not able to carryout during this internship into the next internship.
