week 1
During my first week, I was introduced to my mentor nurse and my mentor nurse teach me about the rules, how to deal with patients, what kind of patients are living in my internship place, dining room, pharmacy, bedrooms, washing places, and so on. I learned a lot of things and I find many differences from my country. I also went to introduce myself to the patient and I helped them with feeding.
Feeling
I felt that I was welcomed by everyone although I don’t know the language some of my colleagues are very good at English. I felt that I can learn so many things from my mentor, my colleague, and the patients.
Evaluation
In my first week, it was a little bit challenging for me because I don’t know how to start work and what to do, I was still confused about the room number, and patient name but I try to remember as much as I can or I ask with my mentor if there is something that I can do or learn.
Analysis
I know this is a new environment for me there are so many things that I don’t understand but, there are so many things that I have to learn during the time I try to ask questions as much as possible.
Conclusion
It feels so good to be a part of this internship. I am realizing that everything takes time we we have to do it one day at a time and things will get better. for me to learn different kinds of things is most important.
Action plan
– I try to communicate with patients in Finnish although I don’t know the language I try to communicate with my little Finnish.
-Asking questions and feedback as much as possible so I can work better in the future.
Week 2
Description:
During the second week,i became used to with the environment especially with the residence and some of the staff members as well.I gave the medication to the patient with the supervision of practical nurse.i communicate with some of the residence in english i gave them moring care such as changing clothes,diapers,combing their hair,giving them shower and so on. one of the residence had passed away while i was in my internship i observed how to take care with the dead body.as well as i painted some of the residence nails comb their hair and so on.i also had a mid term evaluation with my teacher and mentor.
Feeling:
I felt better in knowing the work place because of the help of staff workers here,i think hygine and health is something that we have to pay attention to the residence.we cannot force them to takecare for them but we can pursue them in polite way,for example we can tell them the importance of hygiene and medication and how is the hygine is important and useful for their life.
Evaluation:
As i said during my second week,i learned alot about the care,hygine and communication with residence and i also saw how to takecare of the residence when they expired.
Analysis:
I felt like taking care of elderly patient is not easy as much as i thought but its not even that hard because we can learn the things by the time and we get use too with it.we have to be very calm and tell the residence about the importance of hygine and medication for them because these things are very necessary for them.
Conclusion:
Maintaining hygine is the most important part of everybody life and especially for the people with disability and high risk in elderly so that we have to focus on their hygiene.
Action plan:
-Taking care of the elderly people in proper way.
-Doing everything in aseptic techinque.
-Always ask for permission, introduce with them and and explain the procedure.
Week 3
Description:
During the second week, I tried to give medication to the residents. I have learned that before giving the medication we always have to check the patient’s name and the date of the medicine. some medications need two nurses to check for example oxycodone because it is an opioid drug. I also tried to inject insulin and measure the blood glucose in diabetic patients every time before eating.
Feeling:
I felt better in knowing this work because of the help of my co-workers here I have learned so many things and it has been really easy for me to learn the things. medication is something that we have to pay attention. we can not force patients to take medication, but we can ask them in a polite manner and tell them about the benefit of having medicine and how the medication is useful for our health.
Evaluation:
During the third week, i learned a lot about the medication(route,purpose, side effects, allergies, and so on.)some medication have their immediate effects and long-lasting effect. we have to be very careful while providing medication. we have to follow up and give medication immediately and tell the doctor so that they will know and change the medication that is safe for them.I also tried to make notes on what kind of medication so that I can put it in the medication passport.
Analysis:
While giving medication, we have to know what is the best way for the patient(based on 7Rs): right drugs, right dose, right time, right patient, right route, and right technique and we also have to check the normal range and how to adjust insulin and what to do if there is insulin shock or hypoglycemia and what are the signs.
Conclusion:
Giving the medication is one of the main responsibilities of a nurse(from planning medication to preparing following the seven rights, performing hand hygiene, administering medication, doing the medical calculations,s, etc)
Action plan
-Doing medication (name, purpose, side effect, etc)
-Getting involved in injection.
Asking for feedback about medication.
Week 4
Description
I have learned a lot about the ethical aspect and medical records. we have to keep confidentiality and I also learned about empathy. we should try to be empathetic towards their mental health especially elderly people because they tend to get depressed and their emotion has a lot of ups and downs.
Feeling:
I felt that every one is different and unique,sometimes we cant do same approach to every patient.ethical aspects are very much needed.we need to understand what is best way for them and health care workers.
Evaluation:
There are 4 ethical aspects that we have to consider. autonomy, beneficence, justice, and non-maleficence. each patient has the right to make decisions about themselves and we have to respect that, especially in palliative care. for example, the doctor ordered the use of lap belts to prevent falling but on the other side they may feel uncomfortable for patients. so we have to ask permission from the patients and relatives and then follow up whether that is good for them or not.
Analysis
There are some ethical dilemmas and challenging decisions in some situations. but it needs a lot of practice to have a reliable reason for these things. sometimes, ethical theories are not suitable in some kind of situation but it needs a lot of practice to have a reliable reason for situations that involve ethical dilemmas and if we are not sure, we can ask for help from other workers to make suitable decisions.
Conclusion
We don’t just treat patients physically but also emotionally. because they are human beings and everybody needs love and affection so it is very important to be kind to people, especially elderly ones. we have to make assure them that we care about them and we care about their needs.it is very important to for improving their health outcome, calming their anxiety, and building trust between health care workers and patients.
Action plan
-Consider ethical aspects in every situation and apply them.
-Be professional in the workplace
-Be empathetic towards people as well as with the patient.
Thank you, Safalta, for your reflection on your first week of internship. I was glad to read that you find the ongoing internship a good one for you, and you understand it takes time to learn things. As you wrote, the best way is to do it step by step. You can’t know everything at once. Also it is good that you are not afraid to ask questions or feedback to learn more. It was also nice to read that you try to use Finnish with the patients/residents.
We had a brief mid-term evaluation on 15 June at 10 am with you and Kia via Teams. Everything seems to progress smoothly with your internship. Kia pointed out that you are eager to learn more, and you work actively not just with her, but also with other nurses. Your help is important in the unit where you are working, and you are already facing and handling sensitive situations like the resident passing away.
You will go on with your internship until 1 July, and have your final evaluation with Kia in the end of it.
