Multidisciplinary care planning, wound management, trauma‑informed approaches, and leadership in team meetings.

Key Activities

  • Multidisciplinary meetings: Actively participated in two care planning meetings at Solum, sharing clinical observations and proposing practical adjustments to daily routines to support behavioural stability.
  • Wound care: Performed wound assessments and dressing changes using aseptic technique, measured and photographed wounds for documentation, and updated the care plan accordingly.
  • Motivational interviewing: Used brief motivational interviewing techniques during one‑to‑one sessions to support a resident’s sleep hygiene and daily activity goals.
  • Debriefing and supervision: Held structured debriefs with my mentor after emotionally demanding shifts to reflect on ethical dilemmas and self‑care strategies.
  • Language practice and documentation: Continued writing Finnish care notes daily and prepared a short case summary in Finnish to present at the next team meeting.

Learning Outcomes

  • Integrated care planning: Improved ability to translate clinical observations into concrete, multidisciplinary interventions that balance therapeutic, social, and safety needs.
  • Clinical skills: Increased confidence in wound management, including assessment, aseptic dressing technique, and clear documentation of healing progress.
  • Communication and leadership: Practising concise case presentation and ISBAR reporting strengthened my clarity and assertiveness in team settings.
  • Trauma‑informed practice: Applying trauma‑sensitive communication and debriefing techniques helped maintain resident dignity and reduced re‑traumatization risk.

Reflection

This week highlighted how coordinated teamwork and clear documentation directly affect resident outcomes. Preparing and contributing to care plans made me more aware of the practical constraints and opportunities within the SOTE environment. Debriefing with my mentor was essential for processing difficult encounters and for converting emotional responses into learning points. Presenting a case, even in draft form, revealed areas where I can improve clinical reasoning and succinct communication.

Challenges and Next Steps

  • Challenge: Leading parts of the care planning meeting remains an area for growth.
  • Next steps: Finalize and rehearse the Finnish case presentation for the upcoming meeting, continue targeted vocabulary practice for wound care and behavioural interventions, and request feedback from at least two team members after the presentation.
